You must give this permission on the condition that the information on the sign-up form and any information about your participation in the eligible programs provided by officials be maintained by the company as confidential customer account information. You must be aware that pursuant to Section 837.06, F.S., whoever knowingly makes a false statement in writing with the intent to mislead a public servant in the performance of his or her official duty shall be guilty of a misdemeanor of the second degree.
